Man & two ladies Vacheron & Constantin gold pocket watch

Antique gold open face pocket watch.
The 18 kt gold case engraved with a picture of a men and two ladies. Since they look to be the same age the question is what this picture represents. Does the placing of the men above the two ladies represent a hierarchical order? Is it a menage a trois? Is he a Casanova of the e 1900 century?  The picture itself is not erotic so it is likely that these are where real  people in an actual relationship.
